Admission Information
To meet the admission criteria, the patient must:
- Be medically complex yet medically stable and able to participate in three hours of therapy daily of at least two disciplines (physical therapy, occupational therapy and/or speech language pathology) or 15 hours of therapy over a seven day period
- Require 24/7 nursing care
- Have a discharge plan
- Be able to follow commands and actively participate in therapy
- Meet inpatient rehabilitation criteria based on a physical medicine and rehabilitation (physiatrist) physician
- Have a reasonable expectation for improvement
- Require close supervision by a physician
- Be over the age of 18 years old
What to bring:
- Clothes - Please bring at least one week’s supply of clothing. This should be clothing that you would wear on a daily basis at home. If possible, please bring one pair of comfortable shoes with a back and non-skid soles. Many of our patients find it to be a little chilly at times so you may also want to bring a sweater or jacket.
- Toiletries - We can provide you with basic supplies you will need to get ready for your day, however, you may choose to bring your own toiletries from home. If needed, please make sure to bring dentures with adhesive, hearing aids with batteries, eyeglasses/contact lenses, electric razor and/or cosmetics.
- Valuables - Please leave your valuables at home. This includes jewelry, nice clothes, money and items of sentimental value. Hospital staff cannot be responsible for lost or damaged items.
