Your Stay
During the first few days, the team will meet with you and evaluate your current level and work with you to develop a personalized rehabilitation plan. While you are participating in our program, the following information will assist in making your stay successful.
Programs
- Discharge planning - Discharge planning begins on the first day of admission and is discussed frequently during your stay. The goal and focus of the interdisciplinary team is to get you home as quickly and safely as possible. The case manager, in coordination with the physiatrist, is the main team member in charge of your discharge plan.
- Team conference update - Team conference occurs on a weekly basis. During this time, your rehab team meets to discuss your progress, any barriers and discharge planning. After the team meets with the doctor, members of the leadership team will come to speak with you and any persons you wish to be part of your discharge planning to provide you with an update. This is a great time to ask any questions you may have about your medical needs or discharge process. Please check your communication board for the date and time each week.
- Family/home care training - In order to get you home safely, it may be necessary to involve family members, friends or caregivers in some additional training prior to discharge. We encourage these individuals to be present during the training session so that you are prepared to return home and function safely in the community.
- Performance day - Performance day is your “day to shine” on which you perform all tasks as independently as possible. This generally occurs on one to two days within three days of your discharge date.
